Our Case Management program provides personalized support to individuals with developmental disabilities and their families. Support Coordinators work one-on-one with the people we serve to assess needs, advocate for individual goals, and help navigate available resources. Support Coordinators ensure supports are tailored to promote an individual's independence, personal choices, and quality of life.

Outreach services are designed to assist individuals and families who do not have Medicaid by connecting them to community resources and other supports. By increasing awareness of available services, families will know where to turn for assistance through education, collaboration, and community engagement.

Accessing Services

Before an individual can receive case management and outreach services, they must be determined eligible through the Missouri Department of Health Division of Developmental Disabilities.

An intellectual disability must originate before the age of 18. A developmental disability must originate before the age of 22. Intellectual and developmental disabilities must also have serious impairment in two or more major life areas: self-care, communication, learning, mobility, self-direction, and independent living or self-sufficiency.

The individual must also have a lifelong need for coordinated and specialized supports and services.

Types of intellectual and developmental disabilities include autism, cerebral palsy, Down syndrome, epilepsy, head injury, intellectual disability, learning disabilities, and other related conditions.
